Imlunestrant Combo Makes ESR1 Testing a Selection Step
The FDA approval applies to adults with ER-positive, HER2-negative advanced or metastatic breast cancer harboring an ESR1 mutation after endocrine-therapy progression. Guardant360 CDx is the companion diagnostic.

The September 18, 2026, decision turns ESR1 testing into a defined treatment-selection step rather than a solely prognostic or exploratory exercise. The indication is for adults whose advanced or metastatic disease has progressed after at least one line of endocrine therapy and whose tumor harbors an ESR1 mutation detected by an FDA-approved test.
The approval does not extend the combination to every patient with ER-positive, HER2-negative metastatic disease. Hormone-receptor and HER2 status, treatment history, disease setting and the biomarker result all matter. A positive Guardant360 CDx result links the molecular finding to the FDA-approved use of imlunestrant plus abemaciclib.
Who falls within the FDA indication
Eligibility begins with the disease phenotype: ER-positive, HER2-negative breast cancer that is advanced or metastatic and no longer controlled after endocrine therapy. The indication covers adults, rather than only postmenopausal women, although reproductive and ovarian-function considerations remain governed by the full prescribing information.
The required ESR1 mutation is clinically important because ESR1 alterations can activate estrogen-receptor signaling without estrogen, contributing to resistance to aromatase inhibitors. These mutations are often acquired under treatment pressure and may be absent from an older primary-tumor specimen even when they are present at metastatic progression.

Prior exposure to a CDK4/6 inhibitor is clinically relevant but should not be substituted for the actual wording of the indication. The pivotal trial allowed previous CDK4/6 inhibition, and the FDA label and approval notice define any additional boundaries for on-label use. Clinicians should also distinguish ordinary endocrine progression from rapidly worsening, immediately life-threatening disease, a setting generally underrepresented in endocrine-therapy trials.
What the randomized trial showed
The approval was supported by EMBER-3, an open-label, randomized phase 3 trial involving 874 patients with ER-positive, HER2-negative advanced breast cancer that had recurred or progressed during or after aromatase-inhibitor therapy. Participants could have received a prior CDK4/6 inhibitor. They were assigned to imlunestrant, standard endocrine therapy chosen from protocol-specified options, or imlunestrant plus abemaciclib.
Among 426 participants included in the direct combination comparison, median progression-free survival was 9.4 months with imlunestrant plus abemaciclib and 5.5 months with imlunestrant alone. The hazard ratio for progression or death was 0.57, with a 95% confidence interval of 0.44 to 0.73 and P<0.001. Randomization supports a causal conclusion for this trial population and comparison.
The monotherapy comparison adds context for the biomarker. Among 256 patients with an ESR1 mutation, median progression-free survival was 5.5 months with imlunestrant and 3.8 months with standard endocrine therapy. The hazard ratio was 0.62, with a 95% confidence interval of 0.46 to 0.82 and P<0.001.
In the overall monotherapy population, however, imlunestrant did not significantly improve progression-free survival over standard endocrine therapy. Median progression-free survival was approximately 5.6 months and 5.5 months, respectively, with a hazard ratio of 0.87 and P=0.12. That contrast supports ESR1 as a treatment-selection biomarker for imlunestrant rather than evidence that the drug is uniformly superior across all ER-positive, HER2-negative disease.
The combination produced the expected added toxicity of CDK4/6 inhibition. Diarrhea, neutropenia and nausea were among the more frequent adverse events reported with imlunestrant plus abemaciclib. Safety assessment, laboratory monitoring, interaction review and adverse-event management should follow the complete prescribing information for both drugs; the efficacy results alone do not establish suitability for every eligible adult.
How Guardant360 CDx changes the workflow
Guardant360 CDx is a next-generation sequencing assay performed on circulating cell-free DNA from blood. In this indication, its role is not merely to describe tumor biology. It identifies patients whose ESR1-mutated cancer meets the biomarker component of the FDA-approved treatment population.
Timing matters. Because ESR1 mutations can emerge after aromatase-inhibitor exposure, testing at progression can be more informative than relying on archived tissue collected before endocrine therapy. A blood-based assay also avoids an invasive biopsy and may capture DNA shed from more than one metastatic site.
A negative plasma result requires careful interpretation. Circulating tumor DNA levels vary with disease burden, metastatic location and treatment, so a plasma assay may not detect a mutation when little tumor DNA is being shed. A negative result therefore does not always establish that every tumor site is ESR1 wild type. Whether tissue-based follow-up is appropriate should be determined from the current drug label, the diagnostic instructions for use, specimen availability and the clinical context.
The approval also does not make all commercial ESR1 assays interchangeable. For on-label selection, the result must come from an FDA-approved test used according to its authorized specimen type and instructions. Reports should clearly identify the detected variant, assay, specimen and collection date so that the molecular result can be connected to the treatment decision.
Important evidence gaps
EMBER-3 was open label and funded by the drug’s manufacturer, and financial relationships were reported by investigators. Randomization strengthens the comparison, but treatment awareness can still influence adverse-event reporting, treatment discontinuation and some clinical decisions.
Progression-free survival was the central efficacy outcome, while overall-survival data were not mature enough to establish a survival advantage at the primary report. Longer follow-up is needed to determine whether delayed progression translates into longer life or improved quality of life.
Generalizability is also limited for patients with visceral crisis, poor performance status, extensive prior treatment or serious comorbidities that would have excluded them from the trial. The evidence does not answer every sequencing question, including whether the combination should precede other targeted options when a tumor also carries an actionable PIK3CA, AKT1 or PTEN alteration.
Questions clinicians ask
When should ESR1 testing be considered?
Testing is most directly relevant when ER-positive, HER2-negative advanced or metastatic breast cancer progresses after endocrine therapy and another endocrine-based regimen is being considered. Because ESR1 mutations may be acquired during aromatase-inhibitor treatment, a contemporary specimen is generally more informative than an archival primary-tumor result obtained before that exposure.
Does a negative Guardant360 CDx result exclude an ESR1 mutation?
Not necessarily. A plasma result can be negative because the cancer is ESR1 wild type or because too little tumor DNA entered the circulation for detection. The assay’s instructions, disease distribution and availability of metastatic tissue should inform whether an additional FDA-approved tissue-based assessment is appropriate.
Is an ESR1 mutation alone enough to qualify?
No. The patient must also be an adult with ER-positive, HER2-negative advanced or metastatic breast cancer that has progressed after at least one line of endocrine therapy. The biomarker result should come from an FDA-approved test, and clinical suitability for both imlunestrant and abemaciclib still requires assessment.
Did the trial establish an overall-survival benefit?
No mature overall-survival benefit was established in the primary report. The randomized trial showed a statistically significant progression-free survival advantage for the combination over imlunestrant alone, but longer follow-up is required before concluding that the regimen extends survival.
